Objective 1: Reduce the risk factors for poor physical and mental health

Deliver district wide action and promotion of healthy living via the Living Well programme.Smoking:
Reduce the health harms and inequalities caused by smoking in Bradford population.Reduce smoking prevalence to below 5% across the district population by 2030, with a proportionate reduction in the disparity between the most and least deprived quintiles by at least 5% by 2035.
Obesity:
Reduce the prevalence of overweight and obesity across the district population, with accelerated progress among groups experiencing the greatest health inequalities.
Reduce prevalence of overweight/obesity in reception to ≤17% or remain in line with England average by 2035.

Reduce year 6 prevalence of overweight/obesity to ≤30% or remain in line with England average by 2035. [MHCLG]

Reduce adult obesity prevalence to ≤28% by 2035.
Alcohol:
Reduce the prevalence of harmful alcohol consumption and alcohol-related harm across the Bradford District, with targeted action to address inequalities and improve recovery outcomes.
Reduce alcohol dependency prevalence to ≤13 per 1,000 population by 2035

Reduce untreated alcohol dependency to ≤60% by 2035 while increasing treatment access for underrepresented groups.
Diet and Nutrition:
Increase the proportion of the population consuming a healthy, balanced diet in line with the Eatwell Guide and Bradford District Good Food Strategy, with targeted action to reduce dietary inequalities and food insecurity across Bradford District.
Increase to ≥50% of adults consuming 5-a-day by 2035.

Oral health: Percentage of 5-year-olds with experience of visually obvious dental decay reduces and is maintained in line with national by 2035. [MHCLG]
Set out and deliver a leisure offer that provides opportunities for residents to undertake accessible physical exercise. Physical activity:
Increase the proportion of the Bradford population meeting recommended physical activity levels, with targeted action to reduce inactivity and address inequalities in access and participation.
Increase to ≥75% of adults meeting 150+ minutes of moderate activity per week by 2035. [MHCLG]

Increase to ≥60% of children and young people meeting 60 minutes of moderate activity per day by 2035. [MHCLG]
Work as a partnership to set out and deliver a coherent pathway from early help to an acute offer which is resourced effectively. Mental Health:
Improve mental wellbeing and resilience, reduce the burden of poor mental health, prevent suicide, and alleviate mental distress across Bradford’s population.
Increase the proportion of residents reporting high wellbeing scores by 20% by 2035.

Reduce the proportion of residents reporting high anxiety levels to ≤20% or in line with the England average by 2035.

Reduce suicide rate to be in line with England rate by 2035
Deliver Family Hubs across the district and work as partners to maximise their impact.

Develop and make available resources for young people on health and support services.
Children and young people: Early years: Percentage achieving good level of development at 2-2.5-year old review increases and is maintained in line with national by 2035 [MHCLG]

Vaccination rates: Percentage of children receiving one dose MMR is ≥95%

Sexual health: Under 18 conception rate reduces and is maintained below the England average by 2035 [MHCLG]

Objective 2: Prevent and slow the progression of multiple long-term conditions.

High Level Action Measure Target
Deliver disease specific and coordinated high impact interventions, with a focus on areas/groups with the greatest health inequalities (including case finding, screening, vaccination, therapy, rehabilitation programmes, medicines management). Improvement in:
  • Cancer screening (bowel, breast, cervical cancers, targeted lung check).
  • Vaccination rates (COVID-19, flu and pneumonia, HPV).
  • NHS Health Checks (cohorts of interest) and active case finding for CVD.
  • Structured support (cardiac and pulmonary rehab, diabetes prevention programme).
  • Talking therapies & antipsychotic medication management.
  • Reduce permanent admissions to care homes for people in need of adult social care home.
  • Demonstrable models of care that move from single to multi-morbidity prevention and treatment.
Screening and vaccination rates increase and are maintained in line with the England average by 2035.

Objective 3: Progress the development of Integrated Neighbourhood Health and Care services, helping people to stay happy, healthy at home.

Deliver the Neighbourhood Health Implementation Programme in Bradford (and Craven). Ensure that community care is sustained for the majority of people. The proportion of people who received reablement during the year, who previously were not receiving services, where no further request was made for ongoing support reaches and is maintained in the third quartile of the NHS peer group (ASCOF 2A). The proportion of people aged 65 and over discharged from hospital into reablement and who remained in the community within 12 weeks of discharge reaches and is maintained in the third quartile of the NHS peer group (ASCOF 2D). [MHCLG]

Objective 4: Expand inclusive digital care to allow people to be more independent and take control of their health and wellbeing.

Develop and promote accessible resources for service users and carers. More service users and their carers can easily access the information they need to promote independence and control over daily life. Proportion of people who use services who report having control over their daily life is maintained at ≥90% by 2035 (ASCOF 3A). [MHCLG]

Objective 5: Fully embrace prevention as a way to improve health by moving our resources and finances to meet our population health, care and safeguarding goals

Deliver the locality working plan to move and collocate services to locality level. Increase residents’ life expectancy and healthy life expectancy.

Improve safeguarding outcomes where a risk has been identified.
Reduce the Slope Index of Inequality in life expectancy and healthy life expectancy to be in line with or better than England figures by 2035. [MHCLG]
