Healthy and Happy Residents
Health and care is delivered by a group of partners who work together with the aim of supporting good health and wellbeing for the residents of the district. This includes working to prevent people becoming ill; offering help early; providing good healthcare in the best place for people when it is needed and supporting those who need long term care and support. These partners include - GPs, hospitals, community nursing and therapy services, the Council, private providers of care and the voluntary and community sector.
This outcome also relies heavily on the other outcomes being delivered, in particular good employment practices, safer and stronger communities, and a healthy start to life.
Our Partnership’s Five Objectives:
Objective 1: Reduce the risk factors for poor physical and mental health via district wide action and promote healthy living via Living Well.
Objective 2: Prevent and slow the progression of multiple long-term conditions.
Objective 3: Progress the development of Integrated Neighbourhood Health and Care services, helping people to stay happy, healthy at home.
Objective 4: Expand inclusive digital care to allow people to be more independent and take control of their health and wellbeing.
Objective 5: Fully embrace prevention as a way to improve health by moving our resources and finances to reflect our population health and care goals.
